2017 – 2018 Vision Enrollment Information
All full-time medical students and any student enrolled in a CMSRU approved alternate program of study.
ENROLLMENT DEADLINE: Students must submit enrollment and pay the full premium amount by 9/15/17.

Enroll Online using your credit card. You must enroll by September 15, 2017, to be eligible for coverage effective 8/1/17– 7/31/18.
Paper Enrollment
Enrollment form and full premium must be received by September 15, 2017, to be eligible for coverage effective 8/1/17– 7/31/18.

Open Enrollment Periods for Students: If you are a student in the Fall semester and eligible to purchase coverage and you choose not to enroll for coverage before the Annual Enrollment Deadline of *September 15, 2017 you will not be eligible to enroll again until the start of next Fall.
