Welcome to the Student & Dependent Enrollment Center! Here you will find all the necessary tools to enroll in your school sponsored policy.

We suggest you review the certificate for your rates, effective dates, and optional coverages before starting the enrollment process. The plan’s benefits can be found in the Certificate within the Schedule of Benefits section with the exclusions towards the back.

International Students

All international students, regardless of credit hours, are required to purchase this insurance plan on a mandatory basis.


Hard Waiver Student Eligibility

  • All full-time undergraduate students taking 12 or more credit hours


Are automatically enrolled in this insurance Plan at registration and will be able to waive once proof of comparable coverage is furnished during the open enrollment/waiver period.

Students must actively attend classes for at least the first 31 days after the date for which coverage is purchased. Home study, correspondence and online courses do not fulfill the Eligibility requirements that the student actively attend classes.

Hard Waiver Student Enrollment Instructions

Students are required to Enroll or Waive for EACH SEMESTER. If a student waived or enrolled in the Fall, the student must waive or enroll again for the Spring Semester.

Enrollment Deadlines:

  • Fall Deadline Date –9/21/22 – Closed
  • Spring/Summer Deadline Date –2/22/23

Once you have completed the enrollment, an email will automatically be sent to your campus email address notifying you of the enrollment. You can then go to www.firststudent.com, click on My Account and Click on Create your My Account. You will need to create the My Account, to download your ID card.

If you have missed the hard waiver enrollment deadline, but have not previously waived out of the insurance, you will be automatically enrolled in the insurance by your school, after the waiver deadline. Once you are enrolled in the plan, there are no cancellations or refunds.


 

Hard Waiver Dependents Enrollment Instructions

Eligible students who do enroll may also insure their dependents. Eligible dependents are the student’s spouse and dependent children under 26 years of age.  Please see the Enrollment Instructions below.

Deadlines:

  • Fall Enrollment Deadline: 9/21/22 – Closed
  • Spring/Summer Enrollment Deadline: 2/22/23 (Dependents of new Incoming students only)


Enroll by Mail 


To enroll by mail-download an enrollment form and submit by mail using a check or money order. The mailing address is listed on the back of the Enrollment Forms.

The effective date of your dependent’s coverage will be start date of the coverage period you enrolled in.

If you did not enroll your dependent during the annual open enrollment period but your dependent has since lost coverage due to a Qualifying Life Event please contact us within 30 days of this event.


Voluntary Students

I am a Voluntary student if…

  • I am an Undergraduate student enrolled in 1 or more credit hours, attending face to face classes, and paying student fees
  • I am a Graduate student taking 1 or more credits
  • I am a study abroad student

 

Voluntary students are eligible to enroll for the Student Insurance Plan on their own, with a credit card. (Online, correspondence, and home study program students are not eligible.)

Voluntary Student Enrollment

Open Enrollment Periods

Open Enrollment Periods for all Dependents and Students: If you have eligible dependents in the Fall or are a student in the Fall semester and eligible to purchase coverage and you choose not to enroll for coverage before the Fall Enrollment Deadline is *September 21, 2022, you or your dependents will not be eligible to enroll again until the start of next Fall unless you experience a “Qualifying Life Event” during the year.

 

*For new dependents or new or renewing students in the Spring semester, your open enrollment deadline is February 22, 2023.


Qualifying Life Event

Students who did not enroll themselves or their dependent(s) during the open enrollment period may be able to enroll in the policy if they experience a Qualifying Life Event.

Qualifying Life Events include:

  • Acquiring a new family member
  • Marriage/Civil Union
  • Divorce
  • Loss or change of Job
  • Expiration of Benefits


If you and/or your dependents meet the requirements for a Qualifying Life Event, you will need to provide the following:

  • Proof of qualifying life event/proof of involuntary loss of coverage
  • Enrollment Form
  • Payment

All documents and payment must be received within 30 days of the qualifying life event. After 30 days, the student or dependents will no longer be eligible to enroll in the plan, until fall of the following school year.
