2017 – 2018 Dental Enrollment Information
All full-time and part-time matriculated Rowan University Graduate School of Biomedical Sciences students.
ENROLLMENT DEADLINE: Students must submit enrollment and pay the full premium amount by 2/15/17.
Paper Enrollment
Enrollment form and full premium must be received by February 15, 2017, to be eligible for coverage effective 1/1/17– 7/31/18.

Dental Enrollment Form – Rowan Graduate School of Biomedical Sciences
Plan information and an ID card will be sent to you in the first week of the effective date to the address you list on the enrollment form.
Open Enrollment Periods for Students: If you are a student in the Fall semester and eligible to purchase coverage and you choose not to enroll for coverage before the Annual Enrollment Deadline of September 15, 2017, you will not be eligible to enroll again until the start of next Fall, unless you experience a Qualifying Life Event during the year.
*For new students in the Spring/Summer semester, your open enrollment deadline is February 15, 2018.
