Welcome to the ID Card Center! If you have lost or misplaced your ID Card, worry no more! We are here to assist you. Here, you will have instant access to a temporary ID card and have the opportunity to request a new permanent ID card from the comfort of your own computer chair.
Permanent ID Card
If you would like to request a permanent ID card or print out a temporary ID card, please click here to log into MyAccount. This allows you to request a permanent ID card that will be shipped within 24 to 48 hours of your request. While you are waiting for your Permanent ID Cards to arrive, you may also print out a temporary ID card and MyAccount will do all the work for you. This temporary ID will have your name, Policy number, and ID number already included.
Temporary ID Card
If you are in the UnitedHealthcare System, but did not receive your permanent ID card or do not wish to create a MyAccount, please download The Temporary ID Card. This temporary ID card will not have your name or ID number, however it will give your provider our customer service number and our claims address to enable them to process your claim.
Please note: If you are not in the UnitedHealthcare system, a temporary ID card will not enable you to pick up your prescriptions at your local pharmacy.
