STUDENT ENROLLMENT INSTRUCTIONS
If you are a……
Compulsory student you are required to purchase in this insurance Plan at registration unless proof of comparable coverage is furnished. Please contact the Student Accounts Office at your school.
DEPENDENT ENROLLMENT INSTRUCTIONS
Eligible students who do enroll may also insure their Dependents. Eligible Dependents are the spouse and unmarried children under 19 years of age who are not self-supporting.
