2012 - 2013 School Year

STUDENT ENROLLMENT INSTRUCTIONS
If you are a……
student enrolled in 9 or more credit hours you are required to purchase this insurance Plan unless proof of comparable coverage is furnished.  Please see below to enroll.

  • Annual Enrollment Deadline:  9/30/12
  • Spring/Summer Enrollment Deadline:  2/28/13
  • Summer Enrollment Deadline: 6/15/13


DEPENDENT ENROLLMENT INSTRUCTIONS
Eligible students who do enroll may also insure eligible dependents.
