2012 - 2013 School Year

STUDENT ENROLLMENT INSTRUCTIONS
If you are a……
student taking 12 or more credit hours, you are required to purchase this insurance Plan, unless proof of comparable coverage is furnished. Please contact your school to enroll.

  • Annual Fall:  9/30/12
  • Spring/Summer:  2/1/13
  • Summer: Contact your school

 

 

DEPENDENT ENROLLMENT INSTRUCTIONS
Students who do enroll may also insure their eligible dependents.  You must use MY ACCOUNT or an Enrollment Form to enroll them. Please See instructions below:

 

  • MY ACCOUNT


    If you are already enrolled and only need to enroll your dependents, you will need to create a “My Account”. Once you are logged in, you may enroll your dependents by clicking on the “Dependent Enrollment” button on the My Account page. This will allow you to enroll your dependents without being charged a second time for the student premium.

 


 

  • ENROLL BY MAIL
 
To enroll by mail-download an enrollment form and submit by mail using a check or money order. The mailing address is listed on the back of the Enrollment Forms.
