2012 - 2013 School Year

DEADLINE DATES:

  • Annual Enrollment Deadline: 8/31/12
  • Spring Enrollment Deadline: 1/31/13

STUDENT ENROLLMENT INSTRUCTIONS
If you are a……
full-time Domestic Undergraduate student, you must enroll or waive the school-sponsored insurance plan each academic year. It is important for you to make an informed health assessment each year. Make sure you are not left without appropriate health coverage – it could hinder your academic progress.

Important: If you do not enroll for or waive coverage by the designated deadline, you will be automatically enrolled in the school-sponsored plan and charged the applicable premium. Once you are enrolled, the coverage cannot be cancelled except for certain life changing events and only if the policy has not been utilized.

  • If you do not enroll in or waive the student health insurance by January 14, 2013 you may have a hold placed on your account.
  • Students who do not respond by January 31, 2013 will be automatically charged for the student health plan.
  • The College reserves the right to make the final determination regarding eligibility for the initial and continued enrollment in this insurance plan.


ENROLL NOW    IN COVERAGE ON-LINE

 

If you have missed the hard waiver enrollment deadline, but have not previously waived out of the insurance, you will be automatically enrolled in the insurance by your school.

If you have previously waived out of the insurance and would like to enroll, or if your non-school sponsored coverage is being terminated and you are now looking to enroll in the Student Injury & Sickness plan, please contact Riggs, Counselman, Michaels & Downes, Inc. (800) 346-4075

 

 

 INTERNATIONAL STUDENTS
If you are a……
International student or Visiting Scholar you are automatically enrolled in the insurance plan at registration.

DEPENDENT ENROLLMENT INSTRUCTIONS
Eligible students who do enroll may also insure their eligible dependents. You must use MY ACCOUNT or an Enrollment Form to enroll them. Please See instructions below:


 

You will need to create a My Account.  Once you are logged in, you may enroll your dependents by clicking on the “Dependent Enrollment” button on the My Account page. This will allow you to enroll your dependents without being charged a second time for the student.

 

  •   ENROLL BY MAIL


    Download an enrollment form and submit by mail using a check or money order. The mailing address is listed on the back of the Enrollment Forms.


The effective date of your dependent’s coverage will be the date you enrolled on-line or the date we received the enrollment form with full payment in our office
