We suggest you review the brochure for your rates, effective dates and optional coverages before starting the enrollment process. The plan’s benefits can be found in the brochure within the Schedule of Benefits section with the exclusions towards the back.

2013-2014 School Year (Physician Assistants)

IMPORTANT INFORMATION: Open Enrollment Periods for all eligible Students and dependents: If you or your dependent is eligible to purchase the annual coverage and you choose not to enroll  before the Annual Enrollment Deadline, you or your dependent will not be eligible to enroll again until the following school year unless you experience a “Life Status” change during the year.

PHYSICIAN ASSISTANT ENROLLMENT INSTRUCTIONS
If you are a……
Physician Assistant Student you are required to purchase this insurance Plan, unless proof of comparable coverage is furnished.

All Physician Assistant students will have this $1,160.00 charge applied to their billing statements. If you choose to waive the University health insurance plan, this charge will be removed from your bill within ten business days. If you choose to enroll in the University health insurance plan, this charge will remain on your bill.

 

  •  Annual Enrollment Deadline: 5/31/13


If you have missed the hard waiver enrollment deadline, but have not waived out of the insurance, you will be automatically enrolled in the insurance by your school.

Please Note:  If you do not enroll during the Annual Open Enrollment Period, you will not be eligible to enroll until the following school year, unless you experience a Life Status Change.

full-time graduate student taking 9 or more credit hours or undergraduate student taking 12 or more credit hours or any J1 Visiting Scholars you are required to purchase this insurance Plan, unless proof of comparable coverage is furnished. 

All Full-Time students will have this $1,160.00 charge applied to their billing statements. If you choose to waive the University health insurance plan, this charge will be removed from your bill within ten business days. If you choose to enroll in the University health insurance plan, this charge will remain on your bill.

  • Annual Enrollment Deadline:  8/31/13
  • Spring/Summer Enrollment Deadline: 2/15/14 (new incoming students only)

DEPENDENT ENROLLMENT INSTRUCTIONS
Insured students may also enroll their eligible Dependents.  You must use MY ACCOUNT or an Enrollment Form to enroll them. Please See instructions below:

    If you are already enrolled and only need to enroll your dependents, you will need to create a "My Account". Once you are logged in, you may enroll your dependents by clicking on the “Purchase Optional Coverage” button on the My Account page.
