Full Time, Graduate Assistant working 20 or more hours per week or a Graduate Fellowship or Scholarship Student, receiving $10,000 or more from Utah State University or an International Student or a Full Time Graduate Student employed at the Space Dynamics Lab
You are required to have health insurance to attend Utah State University. You must complete an on-line waiver or enroll in the University’s policy, by the published deadline. To provide proof of other coverage or to enroll, you must complete the on-line waive/enroll process by 9/20/09 Once you are enrolled, there are no refunds.

If You Are a...
USU Undergraduate Student enrolled in 6 or more credit hours, attending classes, and paying student fees or a USU Graduate Student taking 1 or more credits or a Study Abroad Student
You may enroll on a voluntary basis. Students must actively attend classes for at least the first 31 days after the date for which coverage is purchased.
