HOW DO I ENROLL/WAIVE?

Matriculated Students:
To complete the on-line application or waiver process, please go to www.stockton.edu/wellness, in the middle of the page hit the “health insurance” link then choose “Health Insurance Requirements, Electronic Application & Waiver forms” and follow the instructions.
 
 
Non-Matriculated Students:
Non-Matriculated students have the option to enroll in the plan by contacting health services to fill out an on-line application. The individual student’s coverage becomes effective on the first day of the period for which premium is paid or the date the enrollment form and full premium are received by the Company (or its authorized representative), whichever is later.
